Heart surgery without the use of cardiopulmonary bypass

For decades open heart surgeries were performed using the a heart-lung machine and ventilation in order to maintain the patient's vital functions. The device maintains the body's blood circulation and oxygen supply to tissues.

One of the achievements of cardiac surgery in Israel is operating on a beating heart. Only the part of the heart which is being operated on is not functioning whereas all other parts of the heart are functioning in normal mode.

Surgery on a beating heart reduces the risk of complications which derived from the use of cardiopulmonary bypass, such as arrhythmias, disturbances of myocardial perfusion and - very rarely - heart failure.

Coronary artery bypass grafting

The main indication for coronary artery bypass surgery is atherosclerosis of the coronary arteries. Typically, in Israel, the initial treatment of coronary atherosclerosis is angioplasty – the mechanical enlargement of the arterial lumen by means of an inflatable balloon. If angioplasty is ineffective, bypass surgery is the next step. Other indications include stenosis (narrowing) of the left coronary artery, stenosis of the coronary arteries, heart valve insufficiency and extensive myocardial infarction.

During the operation, the blocked or narrowed part of a coronary artery is replaced by a shunt. The shunt is formed from one of the patient's peripheral blood vessels (possibly an artery or vein of the upper or lower extremity) as an alternative way of perfusion of the heart muscle. Sometimes full restoration of blood flow requires several shunt grafts.


Heart valves repair and replacement

Four heart valves provide unidirectional blood flow: from the atria to the ventricles, from the ventricles to the aorta and the pulmonary trunk. The tight closure of the valves prevents the blood flowing in the opposite direction. In some pathological conditions, such as congenital malformations, due to an illness or injury - the function of the heart valve may be impaired thus losing the ability to fully close. This allows the blood to seep in the opposite direction through the valve. As a result, the load on the heart muscle increases and heart failure develops.

Valve defects combined with atherosclerosis of the coronary arteries form a particular danger causing a significant blood supply deficit to the myocardium.

Heart valve insufficiency treatment at the Cardiac Surgery Center at HMC begins with conservative measures. However, if they are not enough, surgery is needed. It can be done by the a valve repair or a full replacement with a mechanical or a biological valve.
